Job Summary:
The role of a Field Coordinator is to ensure clients are being taken care of to the standards set by C&H to promote homecare as a viable and desirable alternative to institutional care.  Field Coordinators are expected to retrieve information about client needs and caregiver performance while in the field visiting clients’ homes to help us make the best matches.

Job Responsibilities:
  • Fill in for clients with last minute openings, fulfilling the duties of a companion, homemaker, or personal care assistant depending on the clients’ needs.
  • Take on new clients until a suitable match has been made.
  • Perform visits to new clients within the first 2 weeks of services to ensure we have accurate information on the client and their first caregiver match is going well.
  • Perform home visits as requested by supervisors to clients’ homes.
  • Thorough documentation of pertinent and relevant information obtained during scheduled time with a client, be it home visit or otherwise. 
  • Escalation of any concerns immediately to direct supervisor pertaining to observation or learned information of clients, caregivers or office staff.
  • Follow weekly schedule of visits to check on caregiver attendance, job performance and client well-being based on direction of supervisor with input from scheduling coordinators, new employee assignments, customer complaints or management request.
  • Prepare a weekly report of all day-to-day activities to be sent to supervisor for review.  Include a mileage report if not driving a company vehicle.
  • Develop a working knowledge of company policies and procedures, services provided, and overall philosophy of the company.  Use all available training tools for this purpose including training documents, company websites, or one on one training.
  • All Field Coordinators are required to report any information, good and bad, that you see or hear about clients or caregivers that affects the company or company’s mission.

  • Skills with using different types of lifting equipment such as a Hoyer lift

  • Ability to be a “private detective” to monitor for concerns at a clients’ home and evaluate caregiver performance
  • Ability to adapt to last minute schedule changes
  • Ability to travel to any town within designated work area using own reliable vehicle.

Work Environment and Physical Demands:
  • This position primarily involves field visits to clients' homes, with occasional light office work as needed.
  • This role requires physical fitness to handle tasks such as lifting and transferring clients and using lifting equipment as necessary.
  • The role necessitates travel within the service area of Connecticut (CT).
